I went ahead and double-checked the OOF setting on the default Remote domain anyway, and it is correctly set. He's receiving mail ok, and his mailbox is well under the size quota. If after that it still doesn't work and you want to investigate the MFCMapi tool, let me know and I can assist with that as well. What we eventually did is use the MFCMapi tool to check and see what rules were applied and deleted the corrupted ones. After that I apply Service Pack 3 no issues reported since then.
