THere is not much available about this on the Internet but some articles suggest that this is an issue caused by Blackberry software and Exchange 2007. Upon further inspection we found that the user had tons of rules and we suspect they reached their rule limit and we just didn't receive an error when enabling their Out-of-Office. Thankfully testing and troubleshooting can be easily achieved using powershell. it seems to be the smart host so looks like thats where the problem may be as the error is 550 relay not permitted.

The issue is caused by opening outlook while logged on to your computer as a different user than your email account, even though you enter the correct e-mail account information while Allow none2. Just double confirm, please let the problematic user log into OWA, click Tools->Out of Office Assistant. All regular external mail will flow as expected this issue will only affect MDNs.
